We have identified an issue in a 3rd Party providers network which is delaying SMS messaging and is the root for the MFA errors. We are in contact with the provider and monitoring for resolution

We are currently investigating reports that some of our customers are experiencing Multi Factor Authentication failures when attempting to access MQD. A response team is gathered and actively evaluating the cause.
